Rhys was last seen playing in his grandparents' garden at Celadine Close in Marton, Middlesbrough, on his red and orange bike which has also gone missing.
Police have searched the local area but initial searches have not yet located the five-year-old.
He is described as having blond hair, blue eyes and a medium build.

Tony Moorhead, 52, and Sharon Moorhead were named locally as Rhys' grandparents.
Great-uncle Brian Moorhead, 57, of Middlesbrough, told the Northern Echo newspaper: "Rhys is always around his grandparents home.
"He visits them a lot and they love spending time with him."
Brian said Rhys' mother is Melanie Moorhead, and his father is called Michael.
He added: "Michael and Melanie split up a couple of years ago. The last time I saw him was a couple of months ago, and it was when my brother Tony brought him round to see me.
"He's a lovely little lad and he's always smiling. I didn't know he was missing."
